About this Program

Students of this program will become competent teachers and other possible professionals in the field of physical education, recreation, health, and sports. Focusing on teaching, students will be prepared to become effective agents to transfer the skills in teaching physical education and health including fundamentals in music, arts, and culture concepts to learners in basic and tertiary education.

  • Certificate of Transfer Credentials (Honorable Dismissal) from the last school attended 
  • Informative Copy of Transcript of Records (with school seal and date of graduation) 
  • 2×2 colored picture (any background) 
  • PSA Birth Certificate (Marriage Certificate for married female students) 
  • Certificate of Employment (at least 1 year of relevant work experience)
 

CONTINUING STUDENTS/USJ-R GRADUATES

  • Certificate of Graduation
